Any object extending above the tower requires the FAA 7460-1 approval if it is added to structure after original tower approval.  Obstruction lights should be mounted as high as possible on the structure.  Remember, lightning rods are also included in the total overall height of the structure.  If adding to an existing structure, be sure to have an aeronautical analysis performed to verify that the tower is not already at its height limit.
